How HSA Payments Work
Download the LMN Form
Your provider will need to complete and sign the LMN.
You can access it anytime under the ‘Forms’ tab or use the button below.
2. Have Your Healthcare Provider Complete It
Any licensed provider may fill it out, for example: MD, DO, DPT
They will include diagnosis codes, medical justification, session duration, and their NPI number.
3. Pay With Your HSA Card
Once your LMN is signed, you may pay for eligible services using your HSA card.
Forged Kinetics is classified under MCC 8099 (Medical / Sports Medicine / Misc. Health) enabling compliant HSA transactions.
4. Begin Your Medically Guided Training Plan
Your sessions will support the goals outlined in your LMN, such as
