Certain services provided by Forged Kinetics may be eligible for reimbursement through a Health Savings Account (HSA) when supported by a Letter of Medical Necessity (LMN) from a licensed healthcare provider.

Forged Kinetics operates on a private-pay basis and does not bill insurance. HSA eligibility and reimbursement decisions are determined by the client’s HSA administrator.

Letter of Medical Necessity (LMN)

An LMN is required to support HSA reimbursement. The form must be completed and signed by a licensed healthcare provider (e.g., MD, DO, DPT).

The LMN typically includes:

• Medical justification

• Diagnosis codes

• Recommended duration or frequency

• Provider signature and NPI

LMN documentation is available under the Forms tab or upon request.

Use of Services with an LMN

When supported by an LMN, services may be used to support goals such as:

• Improving mobility and movement tolerance

• Addressing pain or limitations

• Rebuilding strength following rehabilitation

• Supporting metabolic or functional health

Forged Kinetics does not diagnose or treat medical conditions.

Reimbursement Process

If an HSA card is declined:

1. Clients may pay out of pocket

2. Submit the LMN and invoice to their HSA administrator

3. Reimbursement is subject to approval by the administrator
